The one suture of the surgical incision became loose allowing vitreous leak and prolapse of the iris through the open wound. A secondary IOL implantation surgery is performed to reposition the prolapsed iris and place a new IOL within the anterior chamber. Excessive handling of the eye caused bloodstaining of the inferior half of the cornea. Excessive fibrin formed around the ACIOL and still there is presence of vitreous element within the anterior chamber.
