There is a hematoma within the pericardium near the aortic valve where the dissection of the inner layer of the aorta began and ruptured the aortic wall at the level of the arch of the aorta. Blood flows out from the aorta into the pericardial sac and into the left pleural sac resulting into cardiac tamponade and hemothorax
